Health Kaki - A GenAI-Powered Resident Health Companion

From Healthcare to Health: Putting Humanity Back into Healthtech

Healthcare is evolving into health, shifting from episodic physical “˜sick care’ to a holistic spectrum of well-being, with a greater emphasis on preventative care and healthier living.

Under HealthierSG, health is no longer only about hospitals, large machines and physical encounters, though these will remain a core part of our health delivery. It has now expanded to include interventions and tools that promote health and well-being awareness for residents: empowering and enabling individuals to make positive choices and healthier micro-actions every day. Numerous studies show that these small positive actions, consistently undertaken compounded over time, create new, healthier habits that not only extend lifespan but improves healthspan.

Health Kaki is a promising step forward in realising HealthierSG’s vision by creating a personalised, interactive platform that empowers residents to lead healthier, longer lives.  Under HealthierSG, residents are given health goals by GPs along with a Health Plan, such as “reduce sugar intake.” However, residents often struggle to follow through, as they may not know how to translate this important guidance into action —whether that’s understanding how much sugar is in certain foods and drinks or perhaps simply forgetting to apply the guidance after the consultation. On the flip side, GPs are challenged as well because they often lack the time to provide such detailed guidance or coaching, especially in the context of individual preferences and lifestyles (e.g., vegetarians).

Health Kaki addresses these challenges by offering a “˜digital companion’ that provides residents with personalised information and recommendations tied to their health goals, such as recipes or exercises, after their GP visit. Thanks to GenAI and human-centred design, these recommendations are personalised to each resident’s unique needs, including their health condition, food preferences, allergens, etc. Health Kaki puts that personal human touch back into healthtech. In Temus’ view, the future of healthtech will be increasingly interactive, personalised, immersive, and embedded into daily lives.

 

What is Health Kaki?

Health Kaki, a collaboration between the Ministry of Health, Health Promotion Board, Temus, Amazon Web Services and Synapxe, is a GenAI-enabled prototype that delivers personalised and actionable recommendations based on health goals discussed with family doctors during their HealthierSG consultations.

The Health Kaki proof-of-concept was unveiled in September 2024 at the launch of Synapxe‘s new Innovation Lab, Terrarium, which marked the official opening of the national HealthTech agency’s new location at Elementum in One-North.

Health Kaki is a scalable, modular platform designed to deliver personalised health recommendations. The platform generates tailored diet and exercise plans based on the residents’ health goals, doctor’s notes, and personal preferences. These recommendations are sourced from reliable and validated public health data. Furthermore, residents can interact with Health Kaki to refine and personalise their lifestyle-prescribed action plans.

  • Personalisation: Validated by residents and clinicians, Health Kaki delivers highly personalised health plans that cater to individual needs.
  • Chat feature: A chat function that allows residents to ask questions about their health, health plans or general health information.
  • Integration with Off-line Programmes and Events: Provides information and access to relevant activities at local community centres.
  • Leveraging Public Health Resources: The platform uses trusted, validated resources from Singapore's public health ecosystem, ensuring that recommendations are based on accurate, local health data.
  • Scalable Platform: Health Kaki is built for scalability, offering numerous customisation possibilities, and enabling future expansion into other health domains.
  • Modular Architecture: Featuring an HCC-compliant architecture, the platform's modular components can be extended to incorporate additional functionalities, such as mental health or sleep tracking.
  • Innovation Delivered with Speed and Quality: The platform was developed using a consulting-led, design-centered approach, ensuring both speed and quality in its delivery, thanks to the collaborative efforts of the MOH, Synapxe, and Temus teams.

Health Kaki represents what’s possible when innovation meets real-world needs. By combining advanced AI with a consulting-led and user-centered design, residents can expect to make realistic lifestyle choices and create healthier habits, just like taking the advice of a reliable companion. Health Kaki shows how technology contextualised right, and alongside trusted partnerships, can truly enhance preventive care. We are putting humanity back into health and we’re excited to see how the platform evolves to meet the growing needs of our community.

How Did We Do It?

This POC undertook a consulting, design-led development process with iterative user validation. The development adopted an agile approach with iterative sprints for prototyping, user testing, and technical validation. Feedback from residents and clinicians was gathered to refine the platform, focusing on key parameters such as appropriateness, usefulness, personalisation, and the actionability of the recommendations. Additionally, GenAI assessments helped ensure that the platform could provide relevant, actionable, and safe recommendations.

How Did We Do?

Health Kaki successfully met its goals during the POC stage. Both residents and clinicians gave favourable ratings across dimensions such as appropriateness, actionability, and personalisation. Health Kaki’s AI also performed well in generating accurate and personalised recommendations. Claude 3.5 Sonnet, the AI model used, excelled in various technical metrics, including function calling and instruction following.

What's Next?

The potential for Health Kaki is exciting. Additional features could be introduced, such as nudges or reminders for appointments and screenings, as well as push information about new subsidies or programmes to residents. New health domains, such as sleep or mental health, can also be added. Even more exciting would be adding additional mediums (e.g. videos), integration with wearables, and connections to offline programmes and offerings across the ecosystem. The future of vision is a health companion for the resident embedded in the flow of their life:  knows me, personalised for me, with me on a daily basis.

AI-driven health recommendations also emphasise the need for greater transparency and education about data sources to build user trust in the platform.

Should this platform expand nationwide, increasing data diversity from both local and global sources will further enrich recommendations.
